The Telecommunications Service Request (TSR) Form is now available on-line. Access it by clicking on the "Admin" button and then on "Forms." Use this form for any service your department requests from Telecommunications Services---large or small:

installing, changing or moving a telephone or data line
changing telephone instrument
obtaining or changing a telephone cord or handset cord
adding or upgrading voice mail service
etc., etc., etc.
